Consent Form for Spa Services
I, the undersigned Client, understand that the services provided in the spa promote health and wellness and offers treatments that may provide benefits for various conditions. I acknowledge that while therapists strive to minimize risks, these treatments do not guarantee cures. By signing this form, I agree to abide by the rules and regulations of the spa and Ethiopian Airlines. I confirm that my needs have been acknowledged and that the spa is committed to providing exceptional service.
I consent to the following points:
Purpose
This consent form is designed to ensure that clients understand the nature of the spa treatments provided, acknowledge any associated risks, and agree to the terms of service.
Client Acknowledgment
By signing this document, you acknowledge and agree to the following:
- Authorization of Treatment: I authorize the spa to perform the treatment or procedure I have selected.
- Legal Age and State of Mind: I confirm that I am of legal age and not under the influence of any substances at the time of signing.
- Use of Products: I authorize the use of lotions, oils, and ointments during my treatment and accept full responsibility for any allergic or adverse reactions.
- Professional Boundaries: I acknowledge that the therapy has no sexual intent, and any physical contact with the therapist is strictly prohibited.
- Release of Liability: I release the spa from any responsibility for accidents, illnesses, or injuries that may occur during my visit.
- Accuracy of Information: I agree that all information provided in this form is accurate and true to the best of my knowledge.
- Medical Conditions: I confirm that I do not have any existing medical conditions that could affect the spa treatment.
- Understanding of Procedures: I have read and understood the statements above, and the staff has explained the procedures to me.
- Health Awareness: I confirm that I do not have any terminal illness that would prevent me from receiving spa services.
- Payment Acknowledgment: I understand that payments will be made from international accounts, with reports issued in Ethiopian Birr (ETB).
- Communicable Diseases: I confirm that I am not aware of having any communicable diseases.
- Use of Facilities: I agree to make proper use of all shower and spa utilities.
- Pregnancy Confirmation: I confirm that I am not pregnant.
- Use of Lockers: I hereby consent to use the lockers provided, along with the keys issued by reception, and acknowledge my responsibility to ensure that all personal belongings are securely locked away during the service.
- Personal Belongings Outside Lockers: I understand that the spa accommodates a high number of walk-in guests, and therefore, it is my responsibility to take care of and secure any personal belongings that are outside of the locker.
- Lost Items Policy: The spa will handle all lost items in accordance with established procedures, including reviewing CCTV footage and documenting the location and time items are found. Any items left behind will be taken to the Ethiopian Airlines Lost and Found office, where they will be declared for the owner to retrieve at their convenience.
- Lost Item Responsibility: The Be and Wellness Spa establishment shall not be held responsible for any lost items once they have been declared to the Ethiopian Airlines Lost and Found.
Spa promotes health wellness and healing. There are a lot of medical claims that spa treatment will help in many kinds of medical conditions. There are still risks in this type of treatment and the therapist will do their best to minimize those risks by focusing on what the client demands.
There are a lot of benefits that you will get from this type of alternative medicine. However, we would like to inform you that these treatments don’t claim any therapeutic nor cure assurance. To certify both parties meaning service provider from here and on referred to as “the spa” and service receiver from here and on referred to as “the Client” the spa would like to get a signed consent from the client. By signing this document the client acknowledges and agrees to abide by the rules and regulations of the spa and airlines. By signing this contract the spa acknowledges the client’s needs and promises to provide the outstanding service.
The client consents to the following points
- I authorized this spa to perform the treatment or procedure I’ve selected
- I confirmed that I am of legal age and I’m not under the influence when I signed this consent.
- I authorize the use of lotion, oil, and ointments to my body and will take full responsibility for allergic or any other reaction to the ointments.
- I acknowledge that this therapy has no sexual intent and touching the therapist is strictly prohibited
- I release this massage spa clinic/center for any responsibility in case of an accident, illness, or injury.
- I agree that all information listed below or from another form is accurate and true
- I confirmed that I do not have any existing medical conditions that can affect the spa treatment.
- I confirmed that I have read the statements above and the staff explained the procedures to me.
- I confirm that I don’t have any terminal illness that wouldn’t allow me to take the service provided in the spa.
- I confirm that I am well aware of payments will be made from international accounts but the report will be in Ethiopian Birr (ETB).
- I confirm that I have no communicable disease I’m aware of.
- I agree to make proper use of all shower utilities.
- I confirm that I am not pregnant.
Dispute settlement Governing law and jurisdiction
This agreement shall be governed by the applicable laws of Ethiopia
Any dispute related to this agreement may be settled amicably through mutual negotiation of parties
When the parties fail to settle their dispute amicably, they have the right to refer to the matter to the competent Federal Courts of Ethiopia having jurisdiction based in Addis Ababa.
